2021-2022 Re-Entry

5-11 Year Old Vaccines

A COVID-19 vaccine for children 5-11 years old will be available soon. The Department of Health urges parents/families to register their young children now at vaccinenm.org to facilitate the process when the vaccine is fully approved for this age group. Parental consent forms will be available on the registration website by Wednesday to allow parents to provide consent for their child to receive the COVID-19 vaccine. Check with individual providers to determine if they will accept the form alone or require a parent or guardian to accompany the child in-person.
